Education and training services to monitor, motivate, empower and support a patient's continuous quality improvement program in relation to the management and prevention of coronary heart disease and other chronic diseases

Health services being: assessment of patient's risk factors for coronary heart disease and other chronic diseases; provision of information about the nature and management of coronary heart disease and other chronic diseases; empowerment of patients so they can implement favourable behaviour modification to achieve their risk factor targets by means of a program of continuous quality improvement; and monitoring the patient's progress towards achievement of their risk factor targets and revising the plan of action and providing further monitoring until the target levels for their risk factors are achieved
